2025 Summer Swim Team Registration

Welcome to the 2025 Swim Season (April 14 - July 27). This registration will allow you to register all your children for the Forest Park Swim Team or the Mini Flyers.  Details about working requirements can be found here. Please reach to us either at the pool or through the Contact Us form if you have any registration related questions.

If your child is a mini-flyer volunteer or a swimmer coach, they can be registered separately through a link that will be emailed to you individually.

Is this your family's first year on the Forest Park Swim Team?

Splash Fee

Check this box if you do NOT have a separate swim club membership. Non-members get charged a splash fee to contribute to the upkeep of the physical pool.

Fundraising Fee

Because the cost to run the swim team exceeds the amount collected from swim team dues, an additional
$150.00 of fundraising must be earned for each swimmer under 15 up to a maximum of $450 per family. Fundraising
applies to pool and associate member swimmers. Fundraising dues are collected at registration and are returned
to families if fulfilled. Mini-fliers are encouraged but not required to fundraise - once you reach the invoice page look for the code to remove the fundraising fee for your Mini.

The deadline for fundraising is July 15th. Fundraising opportunities are briefly outlined
below, but more information will be available as the season progresses.

  1. SWIM-A-THON: This is our biggest fundraiser and all swimmers are encouraged to participate! All
    participants will receive an incentive prize for participating, and additional prizes based on the total
    amount raised for each swimmer. Each swimmer collects sponsorships from friends and family and then
    swims laps for a specified period of time based on their age. At the end of the event, funds are collected
    from sponsors. All funds will be applied against a family’s fundraising obligation.
  2. Raise Right: Raise Right is an app that allows participants to purchase gift cards/scrip from hundreds of
    retailers, and a percentage of the purchase is donated back to the team. The app can be downloaded
    from the Apple or Play stores. The enrollment code for FPST is: H19GLU75LSCQ

Full Release and Indemnification Agreement

The undersigned parent and/or guardian consent to his/her child(ren) participating with the Forest Park Swim Team and release the Forest Park Swim Team, Forest Park Swim Club, it’s members, officers, employees, or Board of Directors, as well as the League Board of Directors and volunteers from any liability or claim resulting from any accident or injury sustained by the child(ren) during or coming or going from said team; and I further agree to indemnify and assume all expenses, costs, attorney’s fees, and losses arising from said injury or accident to said child(ren) and to hold said Swim Team, Swim Club, it’s members, officers, employees, and Board of Directors as well as the League Board of Directors and volunteers free and harmless there from.

Family Participation Agreement

Each family with a 14 and under swimmer will have to fulfill work obligations. The amount of these obligations varies depending on the size of the team and will be announced at a later date. It will likely be somewhere between 5 and 8 meets or special events. 

To ensure that each family fulfills their working obligation we require a  $600 deposit check that will not be cashed unless you fail to fulfill the working obligation.

From this check a penalty will be collected as follows:

  • First missed family work obligation - $100
  • Second missed family work obligation - $200
  • Third missed family work obligation - $300 penalty and family swimmer(s) cannot race. 

You must check in at the job desk 10 minutes before your shift or you will not get credit for your job. Two late check-ins will be considered a missed job. 

If you sign up for a job, it is your responsibility to find a replacement if you cannot make it. We aim to have a list of substitutes that you can reach out to.

Photo Release

This is to acknowledge that I am aware that parents are taking pictures during swim events. Pictures may be used on Forest Park's Swim Team/Club’s website and on other promotional materials; however Forest Park Swim Team/Club will not associate names with pictures.

SafeSport Limits on Photography

All video and photographs taken during practice or at any meet this season must observe the generally accepted standards of decency, in particular:

  • Photographs or videos should not be taken from behind swimming blocks at the start of a race or exhibit a child climbing out of the swimming pool.
  • Action shots should be a celebration of the sporting activity and not a sexualized image in a sporting context.
  • Action shots should not be taken or retained where the photograph reveals a torn or displaced swimsuit.
  • Video and photographs should not be taken in locker-rooms or bathrooms.

Parent Acknowledgement of Swimmer Code of Conduct

The Forest Park Swimming Association and Swim Team fosters a family-friendly environment for all members and guests.

In registering my athlete(s) to participate on the Forest Park Swim Team, I agree to ensure all members of my family abide by the guidelines outlined below, in addition to those established by the pool.

  • Swim for enjoyment, not to please others.
  • Treat practices, meets, and activities as opportunities to learn.
  • Represent FPST with excellence, respect, politeness, and team spirit.
  • Follow instructions from coaches, officials, and chaperones. Disrespect will not be tolerated.
  • Exhibit good sportsmanship toward teammates, coaches, competitors, and spectators at all times.
  • Be punctual for all practices, meets, and team events. Pool time is valuable.
  • Attend team meetings, practices, and meets unless excused by staff.
  • Notify the coach in advance if leaving practice or a meet early.
  • Wear appropriate team suits and caps to all meets to show team unity and pride.
  • Commit to giving your best effort at all times, avoiding distractions and cheating.
  • Disruptive behavior or cheating will result in removal from practice.
  • Follow all instructions precisely. If unclear, ask politely.
  • Use respectful language. Profanity or abusive behavior will not be tolerated.
  • Theft, vandalism, and dishonesty will not be tolerated.
  • Avoid associating with any inappropriate or destructive behavior.
  • Alcohol, tobacco, illegal drugs, or banned substances are prohibited.
  • Treat others with respect. Physical altercations or touching will result in severe discipline.
  • Abide by the spirit of the rules. Respect the coach’s directions and be cooperative.

I understand that penalties will be enforced for Code of Conduct Violations:

  • Verbal warning issued.
  • Removal from practice or event, with a parent contact.
  • After being pulled from practice, the swimmer must attend with a parent for 4 consecutive days.
  • Continued misconduct results in a 1-week suspension. No prorated fees will be refunded.
  • Serious offenses may lead to immediate termination from the team, with no fee reimbursement.

All swimmers ages 9 and above will also be required to sign a copy of the Code of Conduct prior to beginning the swim season. Swimmers under 9 require a guardian’s signature. A signed Code of Conduct is required for Swim Team participation.

Speciality Training

While most jobs at a meet can be performed with no training, there are a few that require prior training, either through a clinic or through shadowing. These are computer desk jobs, starter referee (the person that blows the start whistle) and the stroke and turn judge. Are you interested in learning how to perform any of those jobs?  We will reach out for more details if you indicate your interest.  

If you are already trained, do not check this box, there will be another opportunity to indicate your qualification later on.

Substitute

While most people try to fulfill their job obligations, life gets in the way. Sometimes people who sign up for a job find themselves unable to perform it. We would like to have a list of potential substitutes who are willing to step in last minute to take on an extra job. Are you willing to be one of these substitutes that we would contact to see if you might be willing to jump in? 

There are no job points awarded for checking this box, but you will get work credit for the jobs you do end up working.
